Welcome to Rooted & Bloom Counseling and Wellness PLLC. This page provides information about your rights as a client, privacy practices, and important policies related to counseling services.

  • Informed Consent

Counseling is a collaborative process designed to support emotional, relational, spiritual, and personal growth. As a client, you have the right to participate in treatment planning, ask questions, and make informed decisions about your care. Before beginning services, clients will receive and review all informed consent documents through the client portal.

  • Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, health care providers are required to provide clients who are not using insurance with an estimate of the expected charges for medical and mental health services.

You have the right to receive a Good Faith Estimate explaining the expected cost of your counseling services before you begin treatment.

A Good Faith Estimate provides an estimate of anticipated counseling costs based on the information available at the time it is created. The estimate is intended to help you understand and plan for the financial cost of care.

Because therapy is a personalized process, the total cost of services may vary depending on your individual needs, treatment goals, and the length of time you choose to participate in counseling. As a result, a Good Faith Estimate is not a contract and does not guarantee the exact cost of services.

If you receive a bill that is at least $400 more than your Good Faith Estimate, you have the right to dispute the bill.

  • HIPAA & Confidentiality

Rooted & Bloom Counseling and Wellness PLLC is committed to protecting the privacy and confidentiality of your health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA).

Information shared in counseling sessions is kept confidential except in situations where disclosure is required by law, including but not limited to:

• Suspected abuse or neglect
• Risk of serious harm to yourself or others
• Court orders or legal requirements
• Other circumstances required by Texas law

A complete Notice of Privacy Practices is provided to all clients before beginning services.

  • Information We Collect

When you visit this website or request services, information may be collected including:

• Name
• Email address
• Phone number
• Information voluntarily submitted through contact forms
• Information provided through intake paperwork and client documents
• Website usage data and analytics

Information may be used to:

• Respond to inquiries
• Schedule appointments
• Provide counseling services
• Maintain clinical records
• Improve website functionality
• Meet legal, ethical, and professional obligations

Your information is never sold to third parties.

  • Secure Communication

Reasonable measures are taken to protect communications and client information. Please note that email and website contact forms are not intended for emergencies and may not be completely secure. Current clients should use the secure Sessions Health client portal whenever possible for messaging, document sharing, and appointment-related communication.

  • Telehealth Services

Virtual therapy services are available to clients located in Texas through a HIPAA-compliant telehealth platform.

While every effort is made to protect the privacy and security of virtual sessions, no electronic communication system can guarantee absolute security. Clients are encouraged to participate in sessions from a private location whenever possible.

  • Use of Cookies & Website Analytics

This website may use cookies and analytics tools to improve website performance and better understand visitor engagement.

Cookies do not collect sensitive clinical information. You may adjust your browser settings to disable cookies if desired.

  • Client Privacy Rights

Clients have the right to:

• Access their records as permitted by law
• Request corrections to records when appropriate
• Receive information regarding privacy practices
• Request restrictions on certain disclosures
• Receive a Good Faith Estimate
• File a complaint regarding privacy concerns

  • Cancellation Policy

Clients are asked to provide at least 24 hours' notice when canceling or rescheduling an appointment. Appointments canceled with less than 24 hours' notice, as well as missed appointments, may be subject to the full session fee.

  • Records & Record Requests

Clinical records are maintained in accordance with Texas law, professional counseling standards, and ethical guidelines.

Records are retained for a minimum of seven (7) years following the last date of service and are securely destroyed thereafter.

Clients wishing to request copies of their records should submit a written request. Requests will be reviewed and processed in accordance with applicable laws, ethical guidelines, and professional standards.

  • Complaints & Concerns

If you have concerns regarding services received, you are encouraged to discuss them directly with Michiah McFarland whenever possible.
